Introduction
This guide provides step-by-step instructions on how to use the KoboToolbox form to create a provider and service in Directus as a draft. This FORM has been connected to Direcus using webhooks to instantly connect submissions of the form to Directus to create service as drafts.
Accessing the Form
- Open the Form: Use the provided link KoboToolbox Form to access the form.
- Internet Connection: Ensure you have an internet connection. If not, the form can be used offline and uploaded later.
Filling Out the Form
- Select Project: Choose the relevant project from the dropdown menu.
- Provider Information: Fill in the details of the provider. This includes name, location, contact details, and other relevant information.
- Service Details: Provide information about the service offered by the provider. The information collected in this form should match the fields in Directus, with some fields omitted, to be later filled out by the service mapping officer (SMO) upon review. The subcategory field is not included on the form, this is to be determined by the SMO.
- Submit: Once all the information is correctly filled in, submit the form.
Offline Data Collection
- Use Kobo Collect: If you're offline, use the Kobo Collect app to fill in the form.
- Save Drafts: Save your entries as drafts in the app.
- Upload Later: Once you have an internet connection, upload the saved data to the KoboToolbox.
Post-Submission
- Draft Creation in Directus: Upon submission, a draft of the provider and service is created in Directus for the selected project.
Review Process
- Accuracy Check: Service mappers or designated reviewers must check the drafts for services and providers for accuracy.
- Duplicate Handling: Delete the duplicate provider entry if the mentioned provider already exists in the system.
- Mapping Correct Provider: Ensure the service is mapped to the correct existing provider if a duplicate is found and removed.
Finalizing Entries
- Approval: Once reviewed and verified, the drafts can be approved.
- Integration into Database: Approved entries are then integrated into the Directus database as official records.

- Double-Check Entries: Always review your entries for accuracy before submission.
- Regular Uploads: If working offline, regularly upload your data to avoid loss.

Notes
- Language Selection: At present, all submissions from the form will be passed to Directus as Drafts in ENGLISH. We plan to add a "Service Primary language" form which will be mapped to the different language codes that we have in Directus. Note that the display language right now is available in EN/FR/ES/AR, but we plan to add new languages in the future and can do so upon request from implementing teams.
